Maryn Benefits Health, Dental and Life - Medical PPO Plan Our medical coverage is with MAMSI's health plans. In- network benefits are covered at 100% with no deductible and out-of-network benefits are covered at 80%. - Dental PPO Ruby Dental Plan Our dental coverage is with MAMSI's Dental plans. This traditional dental plan allows you to seek services from the dentist of your choice. - Vision The vision coverage is with MAMSI’s health plan. The eye exam is covered at 100% and out-of-network benefits are covered at 80%. - RX Prescription Benefits PPO Plan and Mail Order The employee is responsible for paying the applicable Copayment when using participating pharmacy and out of network benefits covered 80% - Company Paid Life and Accidental Death & Dismemberment (AD&) Insurance We provide basic death benefit to the Insured’s beneficiary(ies), plus an additional benefit (AD&D) if the employee dies as a result of an accident or suffers a covered loss because certain injuries cause by an accident. - Company Paid Short Term Disability The Short-term disability (STD) insurance provides weekly disability income when insured employee becomes disabled.
